Application forms and information on compensation are available, in Greek and English, at the offices of the department of Social Insurance Services or on its website: http://www.mlsi.gov.cy/sid.

You can either deliver your application form in person or send it by post to the following address: Social Insurance Services (Ypiresíes Koinonikón Asfalíseon) Leoforos Lordou Vyronos 7, 1465 Nicosia, Cyprus.
